Important Dates and Explanation of Events
Forms and Documents - deadline: Sunday, March 1st, 2026
- Confirmation Register Information Form
- Baptism Certificate - Must have information about First Holy Communion. Please inform the RE Office if your child was baptized at St. Philip. This information should also be included in the Confirmation Register Information Form.
- Birth Certificate - If there isn't a copy on file.
- Confirmation Sponsor / Proxy Affidavit - Only one sponsor is needed; if the sponsor is not able to be here on the day of Confirmation, you will need to submit a form for the Proxy as well.
- Fees - $75. This covers the cost of the retainable gown for Confirmation and the Retreat.
- Student Height - Must be provided to the RE Office by December 14th, 2025

- Formulario Información Registro de Confirmación
- Fe de Bautismo - Debe tener la información sobre la Primera Comunión. Por favor informe a la Oficina de ER si su hijo fue bautizado en San Felipe. Esta información también debe agregarse en el Formulario Información Registro de Confirmación.
- Fe de Nacimiento (certificado) - Si no existe la copia en el expediente del estudiante.
- Declaración Jurada de Padrino/Madrina o Delegado de Confirmación - Solo se requiere un Padrino o Madrina. Si esa persona no va a estar el dia de la Confirmación, debe presentar este formulario para la persona que lo va a representar (el Delegado)
- Costo - $75. Cubre el costo de la Toga de Confirmación y del Retiro. Si por motivos financieros no puede cancelar este monto, por favor contacte la Oficina de ER.
- Altura del Candidato - Debe darse a la Oficina de ER antes del domingo 14 de diciembre de 2025
